Please fill out this form as completely and accurately as possible so we can get to know you and your pets before your visit.

*HOSPITAL POLICY*

Dogs must be current with DH(L)PP/C, Rabies & Bordetella and

Cats must be current with FVRCP and rabies TO BE IN OUR HOSPITAL KENNEL

We offer nail trims as a courtesy with a bath.

Anal gland expression can be done at an additional charge of $23.00.

OWNER RELEASE: Thomas Animal Hospital will use all reasonable precaution against injury, escape, or death of a pet. The clinic and staff will NOT be held liable for any problems that develop provided reasonable care and precautions are followed. I understand that ANY problem that develops with a pet while I’m absent will be treated as deemed best by the staff veterinarians and I assume full responsibility for the treatment expense involved. If I neglect to pick up my pet within 5 days of the date below and do not notify you within that time frame you may assume that the pet has been abandoned and I hereby surrender ownership of the pet to Thomas Animal Hospital to do with as you deem best and / or necessary.
